Office Appointments

General

Please arrive at the appointment 10 minutes prior to the appointment time for check in. We can update forms, update phone numbers, and verify insurance at this time.

As we make it a priority to start on time for your appointment, please arrive on time. If you do arrive late, we may need to reschedule the appointment.

Please bring the immunization record to each appointment.

We ask that you please do not eat inside the office.

 

Sick Calls

Please call our office for appointments.

If the visit is for chronic concerns such as asthma, behavioral concerns, developmental concerns, chronic symptoms such as headaches or abdominal pain, please let us know so we can schedule the appropriate amount of time.

If you wish to have a sibling seen, please make each child an appointment so we may be prepared to take the best care of each child.

Immunizations:

DTaP- diptheria, tetanus, and pertussis

Hib- hemophilus influenza type b (cause of meningitis)

IPV- polio

PCV- also known as prevnar, pneumococcal meningitis

HBV- hepatitis B

Rota- Rotavirus (stomach flu)

HAV- hepatitis A

MMR- Measles, mumps, rubella

VZV- chickenpox

Tdap- tetanus, diptheria, pertussis for the older child and adult

Menactra- Neisseria meningitis

HPV- human papilloma virus
