Office Appointment Information

Please log in to your MyCoast account prior to your visit, and fill out any required forms online. This will save you time in the office.

Please arrive 10 minutes prior to the appointment time for check-in. We can update forms and contact info, and verify your insurance information at this time. We will make it a priority to start your appointment on time, and hope that you will, too. While we understand that there will be unforeseen circumstances, we cannot inconvenience other families whose appointments will be affected if you are late. We will try our best to accommodate late arrivals, however please know that there may be times when late arrivals must be rescheduled.

If you are unable to make your appointment, we ask that you reschedule 24 hours prior to your appointment to avoid a cancellation or “no-show” charge. Missed appointments mean lost revenue for the office and delayed medical evaluation for other kids in need.

If the visit is for behavioral concerns, developmental concerns, chronic diseases such as asthma, or chronic symptoms such as headaches or abdominal pain, please let us know so that we can schedule the appropriate amount of time for your appointment.

Siblings need to have their own individual appointments so that we will have adequate time to address your concerns about each child.

Well Child Visits

Please bring your child’s yellow immunization record to every well child visit.
